
NTSB report: poor pilot performance in 1995 Star Princess grounding
On 22 June 1995, the passenger ship Star Princess with 1,568 passengers and 639 crew members ran around in Alaska while under a pilot's direction. The National Transport Safety Board determined that the probable cause of the accident was " the pilot’s poor performance, which may have been exacerbated by chronic fatigue‘caused by sleep apnea."
